PUBLIC RECORDS

Public record requests can be submitted through Administrative Services. Please review the below information carefully for additional information regarding accessing records from Suwannee County Fire Rescue (SCFR).

EMS REPORTS

ATTORNEYS

If you are an attorney’s office requesting medical  information on their client’s behalf, may direct their request to Chart Swap. You can contact them by calling 1-800-814-5339 Option 3. Any requests for medical information will be forward to Chart Sawp. Requests cannot be processed without the proper release authority.

SCFR contracts with EMS MC, for medical billing services and will fullfill any requests soley via www.chartswap.com. This ensures the best protection of an individual’s PHI (protected health information), as well as maintain the highest standards of HIPAA compliance.

Information will not be released to outside parties unless ALL criteria has been met.

Records provided through Sharecare include:

  • Medical transport information
  • Invoice for transport services

If you already have an account with Chart Swap you can request records directly from Chart Swap by going to www.chartswap.com and provide them the following information: Name: Suwannee, and location: 13530 80th Terrace. For any updates on the status of a request, please reach out directly to Chart Swap. SCFR does not provide updates regarding the status of a Chart Swap request unless the party is listed as John or Jane Doe.

INDIVIDUALS

Individuals requesting records related to self:

If you are an individual and would like to request information please direct your request to 1-800-814-5339, Option 1.

Please allow 14 calendar days for your request to be complete. More complex requests may take longer. Some requests may prompt an estimated cost of service, which will be provided prior to the start of the work.

Requesting Medical Records in Person:

Suwannee County Fire Rescue can provide with:

  • EMS/Medical transport reports
  • 911 calls dispatched to SCFR are retained for up to one year

If you are requesting records regarding your own medical transport/EMS report, SCFR requires you to complete the County-approved  HIPAA Authorization of Release form, prior to releasing the records. Please review the instructions for submission prior to sending it to SCFR.

Records listed under a Jane Doe, or incorrect information gathered at the time of transport, may take additional time to research.

INVOICES

For parties seeking an invoice related to transport services, or questions regarding an SCFR invoice please reach out to our authorized vendor at 1-800-814-5339 Option 1.

FIRE REPORTS

INSURANCE COMPANIES

If you are an insurance company requesting an incident report related to a house or car fire, etc., please direct your request to prevention@suwcountyfl.gov and include the date of the incident and location.

INDIVIDUALS

Individuals requesting records related to self-purposes:

If you are an individual and would like to request a fire report, please direct your request to the Fire Prevention Division at 386-208-1484.

SUBPOENAS

Suwannee County Fire Rescue Records Department accepts subpoenas via process server or certified mail. Subpoenas for Deposition not received through a process server or certified mail may be returned to the sender. Subpoenas sent via certified mail or process server are accepted at:
