Suwannee County Fire Rescue is responsible, under the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”), to ensure the privacy and security of all protected health information (“PHI”) that we use or disclose.
The Health Insurance Portability and Accountability Act of 1996 (“HIPAA”) grants individuals the right to access their protected health information (“PHI”) contained in a designated records set (“DRS”). Suwannee County Fire Rescue must afford individuals this right of access in accordance with federal and state law. To ensure that Suwannee County Fire Rescue complies with its obligations, there are certain forms that are required to be completed in order to access Private Health Information by which patients or authorized representatives may request access to PHI.

Suwannee County Fire Rescue has an obligation to afford individuals the right to request an amendment to their PHI in accordance with federal and state law.
The Health Insurance Portability and Accountability Act of 1996 (“HIPAA”) and the Health Information Technology for Economic and Clinical Health Act (“HITECH Act”) grant individuals the right to request that Suwannee County Fire Rescue restrict its use of PHI contained in a Designated Record Set (“DRS”). Suwannee County Fire Rescue has an obligation to abide by a requested restriction in accordance with federal and state law.
